All students enrolling in SMFC need all state-mandated immunizations to be up to date. California schools are required to check immunization records for all new student admissions at TK/Kindergarten through 12th grade and all students advancing to 7th grade before entry. Parents must show their child's Immunization Records as proof of immunization. To ensure your child can start school, please make sure ALL forms and immunizations are completed BEFORE the first day of school.
California Education Code § 48216 requires schools to exclude students who do not meet California's immunization requirements. Parents/guardians will be notified and given an opportunity to provide documentation of required immunizations or a valid medical exemption before exclusion occurs.
In accordance with California Education Code § 48216, students who do not meet California's immunization requirements may be excluded from school, and parents/guardians will be notified and provided two weeks to submit proof of immunization or a valid exemption.
Medical Exemptions
Beginning January 1, 2021, all medical exemptions for school and childcare facilities must be submitted through CAIR-ME by physicians licensed in California. Once the exemption is issued, the physician must provide a copy to the parents to give to their child’s school or childcare facility. The law no longer allows parents or legal guardians to choose an exemption from immunization requirements based solely on personal or religious beliefs.

A medical exemption may be issued in CAIR-ME no more than 12 months before a child first enters a grade span (grade spans are birth-preschool, TK/K-6th, and 7th).
Families who have a medical exemption due to a history of disease, for example from Varicella / Chickenpox, must also follow this process.

Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses (4 doses OK if one was given on or after 4th birthday.

All current and incoming 7th grade students are required by law to have a whooping cough booster (Tdap vaccine) and second varicella vaccine on or before the first day of school. If your child has not had a Tdap vaccine and second varicella vaccine, please schedule an appointment as soon as possible with your doctor and submit documentation to your school office.
